Patient Registration
Your registration will be with the Nuffield Practice.
When you register with us you will be allocated a named GP. For continuity of care, we encourage you to see your named GP each time you come to the surgery. There will be times, however, when he/she is not available to see you and when this happens we will provide an explanation and offer you an appointment with another clinician. If you are not happy with your allocated GP you may request a change and we will endeavour to accommodate your request.
NHS Guidelines say ‘If a patient cannot produce any supportive documentation but states that they reside within the practice boundary then practices should
accept the registration’.
There is no regulatory requirement to prove identity, address, immigration status or an NHS number in order to register as a patient and no contractual requirement for GPs to request this. All asylum seekers and refugees and those who are homeless, overseas visitors, whether lawfully in the UK or not, are eligible to register with a GP practice even if they are not eligible for secondary care (hospital care) services.

You can fill these forms in online, and they will be sent directly to the practice. However please be aware that you will need to come in to sign the forms in person before we can activate your registration.
